Job Description & Requirements Specialty: Hospice Discipline: Nurse Practitioner Start Date: 02/08/2026 Duration: 13 weeks 16 hours per week Shift: 8 hours, days Employment Type: Local Contract 13GY Job Title: Travel Nurse Practitioner – Hospice / Palliative Care (Part-Time) Location: Fresno, CA Contract Duration: 13 Weeks Schedule: Part-Time | 1–2 Days per Week Shift: 8-Hour Day Shifts (Flexible) Facility: Hinds Hospice Discipline: Nurse Practitioner (NP) Specialty: Hospice / Palliative Care Assignment Type: Travel – Part-Time Job Responsibilities Support the Medical Director with Medicare-required face-to-face evaluations for hospice recertifications Conduct home visits including private residences, SNFs, and care facilities Coordinate with Case Managers for patients requiring additional follow-up Provide coverage and rounding in the inpatient unit as needed Deliver clinical education to hospice care team members Provide care to both adult and pediatric hospice patients Required Qualifications Active Nurse Practitioner license Hospice experience required Palliative care experience required Pediatric experience required Comfortable working in home-based hospice settings Schedule Highlights Part-time travel assignment 1–2 days per week Facility is highly flexible and open to schedule customization About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
